How to Patch an Old Patchwork Quilt

I have a patchwork quilt made by my great-grandmother, probably in the late 1940s-early 50s.

It has held up pretty well except for several patches of the same fabric which have split and frayed through to the cotton batting.

How can I repair them?

I thought of patching the patches, but the fabric is old and faded and new fabric would stand out too much.

I am not a quilter, so I need a relatively easy fix if possible.

Maybe I could makes patches of some old linen napkins I have and sort of applique them over the rips?
